Enrolment Terms & Conditions
I/we hereby authorise the Principal or representative to approach this student’s previous school in order to gather information on their school record and to disclose personal information to Government agencies, such as the Ministry of Education, Ministry of Social Development, The New Zealand Qualifications Authority and other relevant institutions.
When a student leaves school contact details may be passed to the Ministry of Social Development (MSD) by the Ministry of Education via ENROL and roll returns. This is so young people who may have difficulty finding future employment, training or further education can be identified and offered support by organisations contracted by MSD to help re-engage young people in education or training when they leave school.
I/we grant permission for the school to take photographs of this student (who will not be named) during school activities and that the school may use those photographs on the Lincoln High School website, school newsletters, as well as in marketing material for the school.
I/we declare that the information supplied is true and correct. I/we hereby agree to observe the following conditions of enrolment:
- I/we will ensure that this student complies with school regulations
- I/we will encourage this student to give their best in school work, homework and all school activities and will endeavour to see that they take personal pride in the school uniform.
